Acumen Integrative Medicine actively practices privacy protection of your health information and adheres to HIPAA regulations (the Health Insurance and Portability and Accountability Act). Acumen and Dr. Bruce Gustafson are committed to protecting the privacy of your medical or health information. We are required by law to maintain the privacy of your health information and to provide you with this Notice of our legal duties and privacy practices.
Right to Ask for Confidential Communications. You have the right to ask that we communicate with you about your health information in a certain way or at a certain location. For example, you may ask that we contact you only at home or only by mail.
Right to be Notified of a Breach. You have the right to be notified if we discover a breach that may have compromised the privacy or security of your health information.
As Required By Law. We will disclose your health information when required to do so by federal or state law. For example, we may share your health information with the Department of Health and Human Services if it wants to see that we are complying with federal privacy law.
Respond to Lawsuits and Legal Actions. As allowed by law, we can share health information about you in response to a court or administrative order, or subpoena.
For Treatment. We use your health information to provide you with treatment or services.
For Payment. We use and share your health information to bill or get payment from health plans or other entities. For example, we give information to your health plan so it will pay for your services.
Individuals Involved in Your Care or Payment for Your Care. We may share health information with your family, close friends, or others involved in your care or payment for your care, such as your MD or other healthcare practitioner, ONLY UPON YOUR WRITTEN & VERIFIED AUTHORIZATION,
Acupuncture services are often covered under private or group health insurance plans, but not all symptoms or conditions that might benefit from treatment are covered. Please verify with your individual policy to determine if these services and/or conditions are covered. Currently, we are an in-network provider with Cigna PPO exclusively. We can provide you with an insurance "Superbill" at your request that you can submit directly to your plan for potential out-of-network coverage.
Once your initial appointment is set, I will send you a link for digital access or to download our required pre-treatment, medical and history forms. If you are not able to complete the forms in our EHR software, you can download and bring these forms filled out to our first meeting.
Payment for services can be made using any of the following methods: cash, check, credit or debit card, Zelle, PayPal or Venmo. If an insurance superbill is provided to you, payment is still due at the time treatment services are provided.
Authorization Forms will be forwarded prior to your initial visit, via our HIPAA compliant EHR platform, or we can do this in person prior to your session. We may ask you to update an authorization for treatment at a later date if conditions or scope of care changes.
